Shunt Evaluation to Guide PFO Closure

51yo gentlemen with platypnea-orthodeoxia syndrome with refractory hypoxemia who was found to have a PFO on TTE and multiple AVMs on CT was referred for PFO closure evaluation. TEE bubble study showed more bubbles shunting from the pulmonary veins in comparison to the PFO with continued significant shunting of bubbles following balloon occlusion of the PFO. Given the PFO was not the principal source of shunting, a permanent PFO closure device was not placed. ​
